Overview

Speech Language Pathologist (SLP) / Speech Therapist is responsible for providing diagnostic and therapeutic services to evaluate, treat, document, and facilitate care for residents with impaired swallowing or communication. The role supports the interdisciplinary team, assists in developing communication and swallowing decline prevention policies, and participates in care planning. Location: 48 Cedar St, Brooklyn, NY 11221. Compensation: $41-$58/hour; $5,000 Sign on Bonus. Job posting last updated: 6 days ago. Responsibilities

Evaluate residents’ functional needs when referred by the attending physician or interdisciplinary team. Follow physician orders, facility policies, and procedures to evaluate and treat residents; complete required documentation for care provided. Assist the Director of Nursing (DON) in establishing communication and/or swallowing decline prevention policies and procedures. Participate in discharge planning when appropriate. Perform diagnostic tests and document evaluations of referred residents in accordance with facility policies and procedures. Provide treatment to help residents maintain or attain the highest level of communication and/or swallowing function within their physical abilities. Recommend and/or provide appropriate treatment, preventative measures, and assistive devices to prevent decline in residents’ function. Educate residents and caregivers to help maintain or attain the highest level of communication and/or swallowing function within the resident’s abilities. Maintain timely and accurate resident reports as required by federal, state, local regulatory agencies and third-party payers. Perform all duties in an effective, timely, and professional manner. Required Education and Experience

A current license as a Speech Language Pathologist in the state of practice. Certificate of Clinical Competence in Speech (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA), or equivalent educational requirements and work experience toward certification. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
