Overview

The occupational therapist (OT) delivers and supervises others in the delivery of occupational therapy services. Upon referral, the OT performs evaluations and develops individual plans of care to maximize functional outcomes, implements the plans of care, reassesses treatment results, and actively participates in discharge planning. Services are delivered within the framework of a team approach, including appropriate disciplines, significant others and the patient. Provision of care for pediatric adolescent orthopedic patients may be required. Coverage of both inpatient and outpatient services may be required at the Ballard and Issaquah clinics. These services are delivered in compliance with ethical standards, policies and procedures, national and professional standards, state practice acts, and current accepted state of the art practice. Responsibilities

Deliver and supervise occupational therapy services and participate in discharge planning within a team approach. Perform evaluations and develop individualized plans of care to maximize functional outcomes. Implement and reassess plans of care to monitor treatment results. Provide care to pediatric adolescent orthopedic patients as applicable and cover inpatient/outpatient services at specified clinics. Adhere to ethical standards, policies, procedures, and applicable state practice acts. Required Qualifications

Graduate of an accredited Occupational Therapy program Washington Occupational Therapist License (upon hire) National Provider BLS - American Heart Association (upon hire) About Providence Swedish
