The St. Charles Health System Pharmacy provides a broad range of inpatient and outpatient pharmaceutical care services to ensure safe and effective medication use. Position Overview
The
Clinical Pharmacist
provides pharmaceutical care as a complete package across the hospital medication process to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers. Responsibilities
Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes, including advising physicians on issues concerning drug therapy, drug toxicity, side effects, and assisting in prescribing appropriate doses. Coordinates activities of assigned pharmacy technicians in preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians\' prescriptions; ensures compliance with federal, state and local laws and regulations pertaining to dispensing of drugs and controlled drugs and maintenance of records. Mentors and trains others in training, including practical experiences. Maintains liaison relationships with medical and nursing staff; provides timely information regarding pharmaceutical supplies, drug usage and compatibility, and regulations including Joint Commission Standards. Conducts and evaluates medication histories, assesses compliance and suggests modifications to achieve desired outcomes; instructs patients in proper use of prescribed drugs and makes Patient Care Rounds with physicians to evaluate progress. Individualizes medication regimens using pharmacodynamic and pharmacokinetic principles; responsible for establishing and continually improving pharmaceutical care within areas of direct responsibility and assisting others in the department. Extracts pertinent information from patient sources for the medical management plan. Role models two-way communication of the organization\'s mission, vision, values, and department strategy. Supports Lean principles of continuous improvement; acts as a champion of change. Provides and maintains a safe environment for caregivers, patients and guests. Maintains professionalism and confidentiality; complies with laws, regulations, policies and procedures; supports corporate integrity and cooperates with investigations as required. Delivers customer service and/or patient care in a timely, efficient and accurate manner. May perform additional duties of similar complexity within the organization as required or assigned. Education, Licensure & Certification
Education : Required : Bachelor of Science degree in Pharmacy. Preferred : PharmD., Completion of an ASHP-accredited PGY-1 residency. Licensure/Certification/Registration Required : Current registration by the Oregon Board of Pharmacy. AHA Basic Life Support for Healthcare Provider certification (not required if working exclusively in Specialty Pharmacy). Preferred : Certification through the Board of Pharmacy Specialties (BPS). Experience
Required : N/A Preferred : Six (6) months or more clinical pharmacy experience. Physical & PPE
