Job Description Summary: Post Acute Physician, Skilled Nursing Facility Physician

Key Responsibilities
Perform rounds at assigned nursing homes: conduct regular in-person rounds at designated nursing facilities in Orange County.
Diagnose and manage chronic and acute conditions in elderly and medically complex patients.
Prevent hospital readmissions: proactively manage patient care to reduce unnecessary emergency room visits and hospital transfers; respond promptly to changes in condition and coordinate with facility staff to stabilize patients on-site.
Educate patients and families / conduct goals of care discussions: provide clear, compassionate education about diagnoses, treatment plans, prognosis; lead goals of care conversations including advance care planning, code status, and palliative care options when appropriate; ensure care plans align with patient values and preferences.
Close gaps in care and ensure quality metrics: identify and address care gaps such as overdue screenings, vaccinations, and chronic disease monitoring; contribute to quality improvement and compliance initiatives aligned with value-based care programs.
Document thoroughly and collaborate with care teams: complete timely and accurate documentation in the EHR, support care coordination and regulatory requirements; collaborate with nurse practitioners, nurses, case managers, and other facility staff; provide oversight and support to NPs and participate in case discussions and care planning meetings.
Qualifications
MD or DO with an active California medical license
Board-certified/eligible in Internal Medicine, Family Medicine, or Geriatrics
Experience in post-acute, long-term care, or geriatric medicine preferred
Proficient in conducting goals of care conversations
Familiarity with SNF regulations, Medicare guidelines, and value-based care models
Strong interpersonal, leadership, and documentation skills
Compensation: $232,506 - $290,632 annual salary & bonus eligible
