Position Summary Accountable for assisting consumers in the Involuntary Outpatient Commitment program, including coordinating patient care, providing psycho-education, linking to treatment and other services & entitlements. Works collaboratively with Emergency Services staff, Central, psychiatry, the court system, and the state system of care. Supports the mission, values, and philosophy of HMC. Primary Position Responsibilities Coordinate patient care for consumers on the caseload throughout their involvement in the Involuntary Outpatient Commitment Program. Review records and maintain standards of HMC, JCAHO, DOH, NJ Division of Mental Health & Addiction Services. Provide high-quality case management and linkage to care. Construct treatment plans in conjunction with the coordinator and psychiatrist for court, and monitor consumer compliance. Develop Wellness and Recovery Action Plans and Psychiatric Advanced Directives with consumers. Qualifications Minimum Education Required:

High School diploma or equivalent Preferred:

Bachelor’s Degree in Social Work, Psychology, or equivalent Minimum Years of Experience Required:

Personal experience with mental health issues License, Registry, or Certification Required:

Valid Driver's License Knowledge, Skills, and Abilities Excellent interpersonal communication skills. Ability to identify and resolve problems. Computer literate and skilled in customer service.
