Establish and maintain positive clinical rapport with patients and their families, providing infusion therapy, education related to self-care and self-administration, and clinical monitoring of patient response to therapy. Assess patient appropriateness for home infusion therapy, including safety of the home environment, caregiver support availability, and patient/caregiver independence in therapy administration and monitoring of response to therapy. Obtain and document patient medical history at the start of care and ongoing, with emphasis on the treated diagnosis. Complete all required clinical documentation thoroughly and timely (nursing plans of care, plans of treatment, visit notes, clinical summaries, medication profiles, discharge summaries) and obtain patient and caregiver signatures on relevant consents and forms. Participate in case conferences as needed to plan patient care with the multidisciplinary care team. Provide skilled care to patients of varying ages and medical conditions as outlined in the plan of care and physician orders. Establish, secure and maintain vascular access devices as prescribed (PIV, PICC, tunneled central venous catheter dressing changes, implanted port access). Administer medications as prescribed, performing therapy-specific clinical assessments per policy, including vital sign monitoring, management of acute infusion reactions, side effects, and lab specimen procurement. Communicate observations of patient response to therapy and overall condition with the care team in a timely manner. Establish collaborative relationships with home health agencies delivering services for the Company, providing agency education and support as requested by the nursing supervisor. Maintain knowledge of infusion therapies and Company policies, procedures and protocols to ensure understanding before initiating treatment. Maintain current nursing licensure in all states served, as required by state boards or departments of health. Comply with all state and federal regulations and laws, including OSHA, HIPAA, and home infusion standards of practice. Complete all training required upon hire and annually per policy, including CPR certification. Serve as a professional representative of the Company when educating external customers, payers and referral sources. Assist in staff orientation and training, serving as a clinical educator and resource as requested. Drive personal or rental vehicle as needed to travel to company offices, patient homes, hospitals, physician offices, HHAs, and ambulatory infusion suites. Required Qualifications
Active and unrestricted RN licensure in the state(s) of practice and others as requested. Current Basic Life Support (BLS) and/or CPR certification, or willingness to obtain prior to starting employment. 1+ years of RN experience in a clinical care setting relevant to infusion therapy (e.g., medical-surgical, ED, ICU, PACU, or similar). Proficient with electronic medical record documentation. Ability to work with an interdisciplinary team including pharmacists, physicians, dietitians, HHAs, and others to support patients and caregivers. Ability to work independently in home or alternate-site settings. Ability to meet flexible work schedule and reliability requirements (attendance, overtime, after-hours on-call). Access to reliable transportation for travel to patient sites. Preferred Qualifications
Experience administering infusion therapy in home or alternate-site settings. Knowledge of Microsoft Office and EMR programs; INS standards of practice. Knowledge of HIPAA, OSHA, and accreditation standards for home health and home infusion. Proficiency with Microsoft Office; strong organizational, critical thinking, and interpersonal communication skills. Proven teaching skills. Pay and Benefits
Pay is based on factors including location, education, work experience, and certifications. In addition to salary, benefits include a comprehensive package, incentive and recognition programs, equity stock purchase and 401k contribution (subject to eligibility). The hourly pay range for this role is $28.27 to $50.48 per hour for full-time employment. We comply with all applicable minimum wage laws.
