Overview
Amerita, Inc. is a leading provider of home infusion therapy. We are seeking a Clinical Pharmacist (PRN) to join our pharmacy team as we grow to become one of the top home infusion providers in the country. The Pharmacist will report to the Pharmacy Manager and work in our city/state branch.
Schedule Monday – Friday, 9:00 am – 5:30 pm

Responsibilities
Evaluate and monitor drug therapy for home care patients in collaboration with physicians, nurses and other healthcare professionals.
Adhere to pharmacy practice policies, state and federal laws, and accreditation standards.
Coordinate new referrals and admissions through collaboration with nurses, customer service and distribution.
Review, receive and verify all new patient orders and document on the respective order form.
Assist healthcare professionals with planning and implementing patient care in the home.
Conduct and document initial and ongoing assessment of patient, drug and disease information.
Monitor outcomes of drug therapy by collecting and interpreting laboratory and clinical data.
Provide consultation and answer inquiries from patients, families, physicians and other professionals regarding drugs and their use.
Maintain thorough documentation in the patient record of all clinical activities and communications.
Process and check all medications, supplies and equipment prior to dispensing from the pharmacy.
Maintain clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in‑services, seminars and conferences.
Compound sterile mixtures and non‑mixtures, including parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit.
Ensure compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and USP 797 and USP 800 standards.
Document all activities on appropriate logs.
Oversee pharmacy technician functions as required by Pharmacy Board rules.
Qualifications
Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
Registered Pharmacist License in state(s) of practice.
Minimum two (2) years of recent experience in an acute‑care setting or one (1) year of recent experience in home infusion therapy.
Experience with compounding infused drugs.
Proven record of evaluating and monitoring drug therapies for home care patients.
Good organizational and time‑management skills.
Participation and membership in professional societies and organizations is desirable.
Excellent verbal and written communication skills.
Flexible schedule with the ability to work on‑call evenings and weekends on an as‑scheduled basis.
0–25% minimal travel.
Physical Requirements Constant sitting and typing on a keyboard; occasional standing, walking, push/pull and lift/carry 10–30 lbs.
