Monday through Friday. Start and end times are to be determined and, once set, will remain the same. Clinic hours are 8 am to 5 pm. Responsibilities
Provide diabetes and self-management guidance and education to patients with prediabetes, type 1, type 2, and gestational diabetes. Develop individualized diabetes education plans for patients collaborating with medical providers and monitor adherence to the care plan. Plans should consider cultural, physical, cognitive, and literacy levels of patients and families and meet evolving needs. Provide nutritional counseling for weight and diabetes management. Use nutrition therapy knowledge to assist with meal planning. Teach patients to manage blood glucose levels and dose insulin using carbohydrate counting. Collaborate with patients and medical providers to develop and monitor a physical activity plan that accommodates patients’ needs and goals. Assist medical providers in assessing the need for, and frequency of, self-monitoring of glucose levels. Teach self-monitoring using diabetes technologies and monitor glucose logs and CGM reports. Instruct patients and family members on diabetes medications, including mechanism of action, administration, safety, and insulin/ injectable meds. Help adjust doses based on glucose patterns, dietary habits, and barriers to care. Work with the care team to ensure access to medications and supplies and help patients keep appointments with team members. Identify social and emotional barriers to compliance; refer to mental health resources as needed. Facilitate care coordination with the diabetes care team, pharmacies, and durable medical equipment suppliers. Assist in reviewing and developing clinical practice guidelines. Serve as a content expert for diabetes education and management. Assist in developing policies and procedures for diabetes and health education guidelines. Support the organization as needed and provide diabetes education and device training for clinical research studies. Oversee, submit documentation for, and monitor TZIELD candidates. Required Qualifications & Experience
Bachelor of Science in Dietetics from an accredited program Licensed in Utah as a Registered Dietitian Certification by the National Certification Board for Diabetes Care and Education is preferred At least 2 years of experience caring for individuals with diabetes is preferred Preferred Knowledge, Skills & Abilities
Professional knowledge as a Diabetes and Health Educator Strong analytical and information-gathering abilities Deep knowledge of diabetes and patient care concerns Ability to design, deliver, and evaluate educational programs and materials Ability to provide diabetes-related technical guidance to patients and the care team Strong interpersonal and communication skills Awareness of cultural, social, and economic factors affecting patient populations and ability to establish rapport Ability to work independently and take initiative Proficiency with EHRs, HIPAA-compliant communication platforms, Microsoft Office, Google Workspace, etc. Equal Employment Opportunity
