Overview Case Manager RN - Casual Weekend Days

This is a casual/as-needed position located at Northwestern Medicine Catherine Gratz Griffin Lake Forest Hospital, providing additional weekend staffing support.

Responsibilities

Develops individualized treatment plans for patients, in collaboration with the patient, their family and the care team.

Coordinates and leads care conferences for patients with length of stay greater than seven days.

Documents unusual occurrences and patient relations issues; makes referrals to risk management, infection control and quality departments as needed.

Assesses patient and family needs for appropriate discharge services.

Coordinates transfer to appropriate levels of care (e.g., skilled care, rehabilitation, custodial and home health care).

Documents discharge planning on the interdisciplinary plan of care, in progress notes and education records.

Maintains current knowledge of community resources to support a continuum of effective services.

Delivers notice of Medicare discharge rights within the appropriate time frames.

Assists physicians in assigning admission status and conducts medical record reviews for admissions/continuation of stay.

Monitors avoidable days and recommends changes in services or processes.

Educates staff on CPG/Standing Orders/Protocols to reduce length of stay and resource use while maintaining quality of care.

Ensures compliance with managed care contracts and length of stay targets while maintaining quality of care.

Develops and educates staff on CPG/Standing Orders/Protocols to support quality care and efficient resource use.

Distributes hospital-issued non-coverage notices.

Monitors documentation for appropriate diagnoses for outpatient/observation testing and queries physicians/APPs per AHIMA guidelines.

Responds to requests by patient financial services for appeal assistance and supports Denials Lead RN on insurance denials for medical necessity.

Completes documentation worksheets and conducts chart audits for CMS standards compliance.

Obtains appropriate documentation to comply with core measure standards.

Qualifications

Required:

Registered Nursing license issued by the State of Illinois.

Bachelor of Science in Nursing.

3 or more years of related clinical experience.

Preferred:

Master of Science in Nursing.

Membership in Professional Nursing Organization.

Certification in Case Management (ACM).

Compensation and Benefits The salary range for this position is $40.50 - $64.80 (Hourly Rate). Placement within the range depends on factors such as relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules in the collective bargaining agreement.
