Licensed Clinical Care Manager role at AIDS Healthcare Foundation Location: Las Vegas, NV Responsibilities
Utilize clinical and mental health expertise to assess patient needs identified in the Comprehensive Needs Assessment and implement an Individualized Care Plan. Collaborate with patients, physicians, and the Healthcare Center team to address medical and psychosocial needs; facilitate referrals and service options; assist patients with understanding eligibility and program enrollment. Coordinate care across Ryan White contracts, ensuring appropriate referrals, follow-ups, and retention efforts to support health outcomes and continuity of care. Develop and monitor Comprehensive Assessments and care plans; conduct ongoing reviews every six months and adjust as needed. Act as a patient advocate for core initiatives and ensure alignment with AHF core values and mission. Qualifications
Master’s Social Worker licensed in the jurisdiction where services are rendered (LCSW or LMSW in good standing). Minimum of 1 year of Case Management experience; knowledge of Ryan White Services and related programs preferred. Five or more years of management experience preferred. Current licensure in good standing; knowledge of Ryan White Act and related contracts preferred. AHF Commitment
