Organization DGIM - Hospital Medicine
Job Summary NOCTURNIST position; employee will work overnight. The Advanced Practice Professional (APP – PA or NP) in acute care collaborates with a multidisciplinary team, functions primarily as a hospitalist in an inpatient setting, manages acute/complex chronically ill patients, and may have outpatient follow‑up responsibilities. The APP demonstrates advanced medical and nursing knowledge, sound clinical judgment, and professional communication skills.
Key Responsibilities
Assessment of Health Status – complete history and physical examination for acute and complex chronically ill patients.
Orders and collects data using appropriate assessment techniques and diagnostic procedures.
First responder for nursing and house staff in patient status changes and procedural interventions.
Risk assessment for medication side effects, immobility, nutrition, fluid/electrolyte, immunocompetence, invasive interventions, and diagnostic procedures.
Coordinate daily rounds, present 24‑hour patient report, collaborate in plan of care, and cross‑cover patient population at night.
Collaborate with interdisciplinary team in diagnoses, diagnostic tests, differential diagnoses, and complications management.
Formulate and implement individualized plan of care guided by evidence‑based practice.
Prescribe therapeutic interventions including pharmacologic, non‑pharmacologic, and surgical.
Perform advanced procedures consistent with privileges (minor suturing, wound management, line and tube placements and removals).
Communicate and collaborate with interdisciplinary team, patients, family, and staff to promote continuity of care.
Document key components via daily progress notes, transfer, discharge summary, and relevant clinic notes.
Demonstrate professional practice behaviors – preceptor/mentoring, education, and instruction of students and staff.
Engage in research, evidence‑based practice analysis, and participation in Grand Rounds and council meetings.
