Overview

Elwyn seeks a Licensed Vocational Nurse to perform general nursing procedures, behavior management, direct care and training as dictated by the individuals’ health and behavioral care needs. The LVN may function as lead staff on the shift and may oversee direct care staff. Full-time & Part-time available. $38/hour. Duties And Responsibilities

Perform all procedures within LVN scope of practice. Assist and teach residents self-care, self-administration of medications, and independent living skills including but not limited to bathing, dressing, feeding, and grooming. Assist and train residents in activities of daily living, personal hygiene, and household activities, including but not limited to grocery shopping, food preparation, serving and clean-up. Implement Behavior Plans and Health Care Plans. May participate as a member of the Interdisciplinary Team in designated aspects of the development and implementation of the Individual Service Plan (ISP) and IPP outcomes, and overall programming for residents. Assist residents in occupational, recreational, vocational, and therapeutic programs. Maintain a safe, sanitary, and therapeutic environment which promotes respect and dignity for residents and protects the privacy, rights, confidentiality, and physical/emotional well-being of residents. Engage residents in activities that provide opportunities for learning and practice of skills tailored to the specific needs of the resident. Assist and teach residents in the constructive use of leisure time. Encourage residents to participate in planned recreational activities. Actively supervise during dining, recreational activities, medical and other appointments. Know and report the whereabouts of the assigned residents at all times. Transport residents in the company vehicle to medical, recreational and/or other appointments. Attend staff meetings including but not limited to in-services and training sessions. Maintain all required trainings and certifications as required by regulations and Elwyn California. Report and document behavior/medical concerns and other special incidents to proper supervisory personnel. Remain on duty until relieved by incoming staff. Consistently demonstrate the ability to maintain quality assurance standards and promote Elwyn California goals and objectives in accordance with the organization’s policies and procedures. Continually work with staff members in creating a professional and amiable atmosphere; encourage and support co-workers to work as a team. Provide ongoing constructive feedback to DSPs and CNAs as needed when acting as the lead staff on shift. Demonstrate the ability to adapt to changes in workload. Assist with health care and/or behavioral needs and document appropriately. Complete assigned work in a timely manner and strive to make good use of time. Give feedback to Program Administrator and/or shift lead regarding any issues affecting home operations and resident care; including immediate notification of any changes in care needs. Seek help and/or guidance from co-workers and/or shift lead as appropriate. Respond to emergencies appropriately as directed by all health/behavior plans, regulations, and/or policies and procedures. Other duties as assigned. Qualifications, Education And Experience

High school diploma or GED. Possess and maintain a valid, active license from the State of California to practice as a Licensed Vocational Nurse or Licensed Psychiatric Technician. Minimum 1 year experience with Human Services field preferred. Must complete a DOJ and FBI fingerprint and criminal clearance and be free of convictions barring employment in this position under applicable statute or regulation. Must have a valid health screening with clear TB test within 1 year prior to hire or no later than 7 days after hire. Upon hire, must complete all trainings as required by applicable regulations, including but not limited to: DSP1, DSP2, First Aid, CPR/AED certifications. Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record.
