Strive’s RN Care Managers are a critical part of supporting patient care for individuals with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). They coordinate services to optimize care delivery, primarily via telecommunication, with potential on-site visits to offices, clinics, or patient homes. This hybrid role includes in-home visits based on patient needs and may involve face-to-face meetings with colleagues and leadership. Reports to the Lead, RN Care Manager. Work with the care team to craft personalized, thorough care plans addressing clinical and non-clinical needs. Conduct regular assessments onsite and virtually to detect undisclosed medical or social issues and changes in the home environment; promptly refer issues as needed. Initiate proactive outreach to patients and/or clinics to evaluate status, offer assistance, and address inquiries promptly. Manage a diverse patient panel, evaluate needs, and adhere to NCQA standards for complex case management. Communicate updates to the care team and family members; identify gaps in patient understanding and provide tailored education and resources. Act as liaison with dialysis facilities, convey care plan updates, and maintain accurate program software records. Generate and evaluate reports using Strive data platforms to identify trends and areas needing focus. Provide culturally sensitive care and foster collaboration among clinical team members to enhance care provision. Utilize clinical judgment, analytical skills, and a comprehensive knowledge of case management programs to deliver high-quality care. Minimum Qualifications
Associate’s degree from an accredited school of nursing. Current Registered Nurse (RN) license in the state of practice. 3+ years combined of related education, experience, or certification. Current BLS or CPR Certification required. Efficient and reliable transportation enabling travel to patients’ homes. Ability to provide in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting. Internet connectivity with minimum speeds: 3.8 Mbps upload / 3.0 Mbps download (latency not specified). Preferred Qualifications
3+ years of experience in nephrology, ER, ICU, Cardiac, or Ambulatory Complex case management nursing. Certified Case Manager (CCM) certification preferred. Proficiency with Microsoft 365 Suite (Word, Outlook, Excel) for a hybrid-remote environment. About You
Experience developing strong patient and family relationships that foster engagement and positive outcomes within Strive Health’s Model of Care. Proactive communication with patients and clinics to assess clinical and non-clinical needs. Proficiency in documenting and navigating electronic medical records (EMR) and care plan management.
