Leadership

Upholds Kaiser Permanente policies and procedures, and applicable laws. Leads the health care team; delegates tasks appropriately and demonstrates accountability. Manages scope of practice and escalates issues as needed. Demonstrates professional, supportive behavior and champions new ideas. Leads and directs others through change; uses effective communication and issue escalation to achieve outcomes. Participates in problem identification and resolution; mentors and coaches team members; shares responsibility and authority; ensures patient care activities are completed. Collaborates with other departments to resolve member problems and complies with standards of practice and regulatory requirements.

Nursing Process

Develops and contributes to individualized care plans reflecting assessment, planning, implementation, and outcomes. Ensures multidisciplinary planning, consultation, and education; plans reflect patient data, age-appropriate care, culture, and acuity. Communicates plans with patients and families; revises plans based on progress and evaluation. Monitors progress, ensures safe medication practices, pain control, and discharge planning; educates patients and families for post-discharge care.

Documentation

Ensures accurate, legible, dated and timed charting; documents nursing processes, interventions, and evaluations; uses computer systems effectively for patient care.

Clinical Outcomes

Discusses findings and progress with physicians and the care team; participates in competency validation and department performance improvement activities. Makes comprehensive nursing decisions based on data interpretation and outcome evaluation.

Workplace Safety

Adheres to safety principles and standard precautions; maintains a safe environment for self and others.

Patient Care Experience

Practices customer service standards; promptly responds to patient requests; protects patient confidentiality. Maintains a clean, safe environment; communicates clearly to patients and families; aims to exceed patient expectations.

Team Commitment

Acts as a collaborative, flexible team member; handles conflicts professionally; supports a learning environment and professional development. Mentors new staff, participates in mandatory meetings, and contributes to unit learning needs and annual education planning.

Fiscal Responsibility

Organizes work to minimize overtime and supports system improvements; uses resources efficiently.

Qualifications

Basic Qualifications

1 year of recent (within the last 3 years) full-time equivalent experience in acute care or ambulatory care. Education

N/A License, Certification, Registration

Oncology Nursing Society (ONS) Provider Certification OR Pediatric Chemotherapy & Biotherapy Provider Certification Registered Nurse License (California) Basic Life Support (BLS) Additional Requirements

PICC didactic course and PICC insertion experience, as applicable. N/A Notes

Weekends and holidays required. May float to Ambulatory Infusion and Ambulatory Infusion/Chemo at Zion (+1); may float to all SDSA locations. Cross-train to Ambulatory Infusion (AIC) and Message Management. Job Details
