Psychiatric Mental Health Nurse Practitioner In this role, you will be assigned to a facility or facilities in your local community, allowing you to build meaningful connections with the individuals you serve. You will conduct in-person psychiatric assessments, medication management, and implement Medicare compliant gradual dose reduction strategies for psychotropic medications. We provide accredited CEUs and ongoing case consultation with our experienced team of clinicians to support your professional development. Responsibilities

Conduct comprehensive psychiatric evaluations and mental health assessments for patients in Long Term Care facilities, ensuring continuity of care.

Develop and implement individualized treatment plans, including prescribing and managing psychotropic medications.

Collaborate with interdisciplinary healthcare teams to coordinate care and optimize outcomes.

Monitor and manage chronic mental health conditions such as depression, anxiety, bipolar disorder, and schizophrenia.

Maintain accurate documentation of patient visits, treatment plans, and progress notes using electronic medical records (EMR).

Qualifications

Current licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP or PMHNP-BC).

A valid DEA Number.
