Position Overview Optum seeks a Nurse Practitioner or Physician Assistant for our Home and Community Care team in Anoka, Hennepin County, MN. The role provides annual clinical assessments in patients’ homes, preventing health risks and coordinating follow‑up care.
Benefits Flexible scheduling – 20 or 40 hours per week, no on‑call, weekend or holiday required. 85% travel statewide, expenses covered. Base salary plus incentives.
Primary Responsibilities
Conduct in‑home assessments: medical history, systems review, medication reconciliation, vital signs, comprehensive exam.
Perform evidence‑based screenings and point‑of‑care tests.
Identify diagnoses for care management and active medical management.
Formulate current and past medical conditions list.
Communicate assessment findings to member PCP.
Identify urgent and emergent situations and intervene.
Educate members on disease process, medication, compliance.
Required Qualifications
Active NP or PA license (or ability to obtain by start date). Unencumbered, no disciplinary actions.
Active ANCC or AANP national certification (Family, Geriatrics, Adult, Adult‑Gerontology Primary Care, or Acute Care) or equivalent. PA national certification through NCCPA.
Ability to complete physical requirements (move 30‑pound bag, navigate stairs, maintain stationary position).
Driver’s license and reliable transportation for home visits.
Preferred Qualifications
1+ years of clinical experience at highest education level.
Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
Home health care or home visit experience.
May need additional licensure in other geographic areas.
Compensation Salary $89,800 – $176,700 annually based on local labor markets, education, experience, certifications. Benefits include comprehensive package, incentive programs, equity stock purchase, 401k contribution.
