Responsibilities The
Physical Therapist (PT, DPT, RPT)
in Home Health evaluates patient care needs related to mobility, endurance, pain, and medical conditions. Based on assessments, the therapist develops treatment plans, performs interventions to improve patient well-being, and monitors progress.
Evaluate patients’ functional status and assist in developing therapy plans with physicians
Observe and report patient responses and condition changes
Treat patients to relieve pain and restore or maintain function
Educate healthcare team members, patients, and families about care plans and progress
Education and Experience Licensure Requirements:
Current Physical Therapy license in California
Current CPR certification
Valid driver’s license, vehicle insurance, and access to reliable transportation
