This role is a key member of the integrated care team, providing short-term, individualized clinical interventions for members with behavioral health conditions such as substance use, depression, anxiety, schizophrenia, or bipolar disorder. The approach emphasizes trauma-informed care, harm reduction, and motivational interviewing. The BHC acts as the behavioral health SME within an interdisciplinary team, which includes primary care providers, nurses, care managers, psychiatrists, and others. Responsibilities include comprehensive biopsychosocial assessments, developing person-centered treatment plans, providing therapy, crisis intervention, and follow-up care post-hospitalization. The role also involves promoting adherence to treatment plans, utilizing evidence-based practices, and maintaining thorough documentation. Key duties include: Providing individual, group, and post-discharge therapy with documented progress tracking. Conducting crisis interventions and de-escalation. Serving as a behavioral health SME, offering recommendations and referrals. Applying evidence-based approaches such as trauma-informed care and motivational interviewing. Collaborating with the care team and maintaining communication with PCPs and psychiatrists. Developing and supporting treatment plans for complex mental health conditions. Promoting medication adherence, including long-acting injectables. Managing appointment scheduling, attendance, and follow-up. Developing group therapy curricula relevant to the member population. Minimum qualifications include: Licensed in the state of practice (e.g., LCSW-C, LPC, LPCC, LCPC, LMHC, PhD, PsyD) with 3+ years of behavioral health experience. At least 2 years of experience in community mental health or similar settings. Experience with complex populations, government programs, and telehealth preferred. Strong clinical judgment, computer proficiency, excellent communication skills, and ability to work independently. Active CPR certification. Working conditions involve a professional office environment with potential for remote work. Physical requirements include effective communication, ability to remain stationary, operate office equipment, and occasionally lift objects up to 20 lbs. No direct reports.
