The Physical Therapist in Home Health assesses and evaluates patients' needs related to mobility, endurance, pain, and other medical conditions. Based on these assessments, the therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.
Responsibilities Evaluate patients' functional status and physical therapy needs, and consult with physicians to develop care plans. Provide services within the scope of practice as defined by state laws, in accordance with the care plan, and coordinate with the healthcare team. Observe and report patient responses and condition changes to supervisors and physicians. Treat patients to alleviate pain, restore or maintain function, using evidence-based techniques. Educate patients, families, and healthcare team members about care plans, limitations, and progress. Visit patients across the service area and nearby agencies as needed. Complete progress notes for each visit and submit them per policy. Conduct evaluations and develop plans of care within state guidelines, reporting outcomes within 24 hours. Participate in staff meetings and committees as requested. Supervise home health aides and PTAs as permitted by state regulation and policy. Participate in weekend rotations to provide services as needed. Prepare discharge summaries and periodic patient condition reports. Provide in-service education as required. Adhere to therapy standards of practice and maintain up-to-date clinical knowledge and licensure. Perform other duties as assigned.
Education and Experience Licensure Requirements Current Physical Therapy license in PA Current CPR certification Valid driver's license, vehicle insurance, and access to reliable transportation
