We provide advanced clinical services and emotional support to patients recovering from illness, injury, or surgery, enabling recovery in the comfort of their homes. Responsibilities

Coordinate, implement, and evaluate the ongoing home care needs of patients. Review patient insurance information and coordinate activities for each care plan, sharing pertinent findings with family and professional caregivers. Utilize excellent communication, documentation, and time management skills to ensure quality care, with autonomy and respect for your opinions. Precept new nurses and supervise peers during field visits. Minimum Qualifications

Graduate from an accredited School of Nursing; BSN preferred. Minimum of 3 years of home care field experience; supervisory or leadership experience preferred. Valid NYS RN (Registered Nurse) license. Valid Driver's license. Oasis Certification preferred. Knowledge of Home Health regulations. Ability to supervise, mentor, and coach home care nurses. Proficiency in computer skills, including EMR and Word.
