Overview
Ambulatory Care Clinical Pharmacist at Trinity Health. Responsible for providing comprehensive medication management services for patients cared for by the Saint Alphonsus Medical Group (SAMG) Clinics and/or patients managed by the Saint Alphonsus Health Alliance (SAHA). Works collaboratively with physicians, advanced practice providers, staff and other health care professionals to provide medication management, education, and care coordination across the health care continuum. Focuses on supporting patients upon discharge from an acute care and post-acute care setting and providing chronic disease state management of high and rising risk patients associated with assigned SAMG clinics, through collaborative practice agreements, and with patients of practices in SAHA under the supervision of the Executive Medical Director Value Care. Ensures excellent patient experience, patient safety, best practices, and high-quality standards of care across the continuum of care. Participates in self-management support and chronic-care management activities for the clinic\'s patient population and supports care coordination and population health management across the system. Responsibilities
Provide comprehensive medication management services for SAMG and SAHA patients; collaborate with clinicians and care teams to optimize pharmacotherapy and improve patient outcomes. Assist in developing and coordinating self-management support and chronic-care management activities for the assigned patient population. Support patients with complex medical needs and extensive medication regimens; address barriers to adherence and medication access; provide education and follow-up between primary care or specialty visits. Contribute to patient experience improvements, patient safety initiatives, and adherence to best practices and quality measures across the care continuum. Support care coordination and population health management efforts across the Saint Alphonsus system. Qualifications
Currently licensed in good standing with the Board of Pharmacy in Idaho; licensed in Oregon within 6 months of hire. PharmD required. Bachelor\'s Degree in Pharmacy with 3 years of experience may be considered in lieu of PharmD. Minimum of 1 year of patient-care experience required or equivalent. Residency training: PGY1 preferred. Board Certification in Pharmacotherapy or Ambulatory Care preferred. Previous experience in both acute care and community or long-term care pharmacy preferred. What You Will Do
Complete generic and department-specific skills validation and competency testing; demonstrate excellent physician communication skills. Apply critical thinking to analyze complex data sets; implement evidence-based interventions and protocols for chronic conditions. Influence and negotiate care decisions; work autonomously and as part of a team; adapt to changing priorities. Foster customer-focused interpersonal relationships with practitioners, the health care team, patients, families, and community partners. Develop positive, longitudinal patient relationships and understand payer expectations; support compliance with payer pharmaceutical management programs and quality measures. Monitor therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions; optimize pharmacotherapy opportunities and formulary adherence. Review clinical paths and order sets; act on automated surveillance and decision-support tools. Document adverse drug events, interventions, and required QARs; provide feedback on clinical pharmacy risk issues and risk reduction. Support compliance with Saint Alphonsus Health System policies, regulatory and accreditation programs; complete annual competencies and licensure requirements.
