A licensed physical therapist who complies with the Code of Ethics/Standards of Practice as outlined by the professional organization and the State of Illinois. The position includes direct patient care, teaching, participation on appropriate committees and department process improvement. Serves as a role model for therapists and clinical interns in professional, clinical, communication, education, and leadership activities. Responsibilities

Demonstrates the four NM values: teamwork, patients first, integrity, and excellence. Presents a friendly, professional demeanor and appearance. Provides accurate information and timely updates to patients and customers; addresses questions promptly and provides directions as needed. Uses effective service recovery skills to resolve issues. Maintains primary responsibility over patient plan of care with collaboration from treating PT/PTA(s). Demonstrates teamwork across departments and communicates effectively while respecting diverse opinions. Uses resources efficiently and manages work schedule to complete tasks on time. Participates in continuous improvement efforts and opportunities to reduce costs and grow services. Adapts treatment plans creatively to special circumstances with team feedback and documentation. Maintains essential skill competencies in evaluation and treatment modalities; performs re-evaluations per department guidelines and documents appropriately. Maintains responsibility over patient management from initial contact to discharge; collaborates with PTA(s) and documents in EMR and notes per requirements. Provides educational materials appropriate to patient understanding; participates in committees and department projects as needed. Develops professional relationships with healthcare team members and participates in internal/external marketing as required. Fosters a culture of respectful communication and receptive feedback; accepts coaching and implements changes. Completes non-patient care responsibilities within established timelines (coverage notes, schedule changes, etc.). Contributes to department educational efforts (in-services, journal clubs, orientation, PI) as expected. Participates in student clinical programs as a mentor or host as per department expectations. Maintains productivity standards and willingness to adjust schedule to meet patient care needs and department meetings. Qualifications

Required:

Bachelor’s Degree in Physical Therapy Licensed Physical Therapist in the State of Illinois BLS from American Heart Association on hire Preferred:
