Responsibilities
Direct patient care and participation on appropriate committees and departmental process improvement.
Serve as a role model for therapists and clinical interns in professional, clinical, educational and leadership activities.
Demonstrate four NM values: teamwork, patients first, integrity, and excellence in daily work.
Provide accurate information and timely updates to patients and customers; address questions promptly and provide directions as needed.
Collaborate with treating PT/PTA to maintain the patient plan of care and ensure continuity of care.
Demonstrate effective teamwork and communication across departments; respect diverse opinions and contributions.
Maintain productivity and manage workload to complete tasks on time.
Contribute to continuous improvement and cost-reduction efforts; participate in service enhancements.
Adapt treatment plans for special circumstances with documented evidence and team feedback.
Complete evaluations, re-evaluations, progress notes, and discharge summaries in the EMR per department guidelines.
Provide educational materials appropriate to the patient’s level of understanding.
Participate in department processes, committees, and special projects as aligned with clinical expertise.
Develop and maintain professional relationships with healthcare team members; participate in internal and external marketing as per department expectations.
Maintain professional conduct, seek and respond to feedback, and support team members as needed.
Complete non-patient care responsibilities within established timelines (coverage notes, PTO/schedule changes, etc.).
Engage in student clinical programs as a clinical instructor, mentor, or observation host as expected.
Maintain productivity standards per department expectations and be flexible to meet patient care needs and department meetings.
Qualifications
Bachelor’s Degree in Physical Therapy.
Licensed Physical Therapist in State of Illinois
BLS from American Heart Association upon hire
Preferred: Masters or Doctoral Degree in Physical Therapy

Hours/Schedule: IHR (In House Registry) to cover PTO, Leave, or Schedule gaps. Expectation 192 hours in a year, including 4 weekend days per quarter and 2 holidays. Monday–Friday weekday availability is preferred.
