General Summary of Position
Job Summary:
MedStar Health is currently seeking an Inpatient Case Manager to join our team at MedStar Good Samaritan Hospital. This position facilitates the delivery of quality, cost-effective, patient-centered care from pre-admission through post-discharge.
As a member of the Case Management team, you will monitor the care and services delivered to selected patient populations during the acute hospital stay, promoting effective case management and utilization of resources to facilitate the continuum of care and achieve optimal clinical outcomes. You will collaborate with multidisciplinary healthcare teams to develop and coordinate care plans tailored to meet individual patient outcomes.

Primary Duties
Develop care plans addressing patient needs across the continuum, setting long- and short-term goals with specific timeframes, and involving family or caregivers. Include contingencies such as power of attorney and advance directives.
Collaborate with community agencies to facilitate referrals based on patient needs, ensuring timely initiation. Use resources to assist patients in resuming community life or adjusting to lifestyle changes.
Facilitate communication among patients, families, and healthcare providers. Assess and communicate social, cultural, emotional, and economic issues. Participate in multidisciplinary rounds and collaborate on discharge planning.
Complete psychosocial or socioeconomic assessments promptly when referred or indicated by high-risk factors.
Maintain updated information on community resources and referral requirements relevant to the patient population.
Qualifications
Master's degree in social work from an accredited school (Council on Social Work Education).
1-2 years of social work experience, preferably in a hospital setting.
1-2 years of care/case management experience preferred.
Licensed Certified Social Worker - Clinical (LCSW-C) or Licensed Independent Clinical Social Worker (LICSW) license in DC or Maryland, depending on work location.
Certified Case Manager (CCM) certification preferred.
Minimum Qualifications This position has a hiring range of $60,632 - $107,494.
