Per Diem Advanced Practice Provider, Lead Care Manager (Per Diem) – Gifted to cover weekends and after-hours as needed. 19 hours per week with weekend shifts: Saturdays 9:00 am – 7:30 pm; Sundays 9:00 am – 6:30 pm. The primary purpose of the Clinical Programs of askMDAnderson is to provide transitions in care and after-hours triage via phone and online communication for patients, family, and caregivers after visiting the MD Anderson Cancer Center. The program supports access to care during evenings and weekends and serves as the front-line healthcare resource for clinical support. This role requires comprehensive knowledge of cancer care, strong communication, motivational interviewing, and familiarity with MD Anderson services and resources. Shifts vary; Per Diem APP covers primarily weekends and after-hours as staffing needs arise. Maintains clinical practice to sustain licensure, certification, and knowledge competency to provide safe, high-quality patient care. Key Functions
CONNECT discharge phone call program: Respond to clinical alerts (medication questions, care instructions, follow-up appointments, post-discharge needs) via telephone or online communications to prevent unnecessary readmissions, ER visits, or caregiver burden. Close care gaps using approved resources and oncology expertise. Proactive assessment to prevent complications and remind clients to complete surveys. Provide quality patient care aligned with diagnosis, treatment, age group, and needs. Communicate findings to physicians and families. Assess health status via medical history, physical examination, and psychosocial assessment. Communicate clearly; document findings in accessible language. Provide autonomous nursing care within scope and assist with understanding and navigating the MD Anderson referral process. Use comprehensive communication and interviewing skills to respond to information requests per askMDAnderson and MD Anderson policies. Utilize standard medical/legal disclaimers to describe professional role; translate technical information into accessible language and employ motivational interviewing. Participate in data collection and management using standard software; document follow-ups timely in CONNECT program and EMR. Handle inquiries from challenging clients with effective communication to reduce anxiety and convey clinical knowledge. Coordinate service recovery and patient advocacy resources; monitor customer satisfaction and needs. Bridge communication between patient/family and interdisciplinary team; coordinate services related to patient care. Provide health education on side effect management and anticipatory guidance. Maintain core knowledge of cancer and MD Anderson resources; stay current with empirical outcomes and institutional best practices. Complete ONCC certification within 1 year of employment; maintain continuing education requirements. Utilize computer and telephony systems effectively; assimilate new information into operations. Identify resource gaps and conduct independent research to address needs; support colleagues and participate in change initiatives. Assist in new employee training and ongoing education as competency is demonstrated. Use data to assess personal performance; align with departmental standards and performance goals. Serve as a resource for certification requirements; provide feedback to the team and participate in quarterly reporting. Quality Assurance / Customer Service: Assess client information needs; tailor responses; verify understanding and satisfaction; document and monitor inquiries; identify opportunities to improve processes and service quality. Team Involvement: Lead projects, participate in Team Anderson activities, and adhere to policies. Support professional development and foster collaboration to improve patient outcomes. Advanced Practice RN Education Required: Master’s Degree in Nursing or Doctor of Nursing Practice. May be waived for applicants recognized as an APRN by the Texas Board of Nursing prior to 1/1/2003. Graduate education must align with clinical practice area. Preferred: ONCC or AANP/AOCNP certifications. Physician Assistant Education Required: Graduate of an approved PA program. Experience
Experience Required: Minimum 2 years oncology experience in hospital or clinic; experience speaking with patients and families by phone; proficient with computers and multitasking. Preferred: 5 years in oncology or population health; bilingual Spanish/English. Licensure / Certification
APN: BON authorization to practice; PA: NCCPA certification and Texas licensure. Required prescriptive authority as applicable. BLS mandatory; ACLS/PALS optional.
