Reports to: APP Clinical Manager

Shift Hours:

19 hours per week. Saturdays, 9:00am - 7:30pm. Sundays, 9:00am - 6:30pm. Weekend Rotation: 5 out of 6 weekends (Saturday - Sunday) in a 6-week schedule. One weekend off in a 6-week scheduled block. Staff will be scheduled during the weekday during their off weekend.

Holiday Commitment:

1 major bundled holiday. 3 minor holidays.

Considered part of the "ride out" team and essential commitment to show up to 1MC.

Position Summary

The primary purpose of the Clinical Programs of askMDAnderson is to provide transitions in care and after-hour triage via phone and online communication for patients, family, and caregivers (hereafter clients) after they have visited the MD Anderson Cancer Center. These programs support patients’ access to care evenings and weekends and serve as the front-line healthcare resource for anyone who needs clinical support. This position requires strong knowledge of cancer care, excellent communication skills, motivational interviewing, and knowledge of MD Anderson services and resources. Shifts vary as Per Diem APP, covering primarily weekends (Saturday & Sunday) including evenings, nights and holidays as staffing needs arise. The APP will maintain clinical practice to sustain licensure, certification, and clinical knowledge competency to provide safe and high-quality patient care.

Key Functions

CONNECT discharge phone call program:

Respond telephonically or via online communication to clinical alerts (medication questions, care instructions, follow-up appointments, post-discharge DME, home health, or resources) to prevent readmissions, ER visits, or unnecessary burden.

Close care gaps using approved resources and oncology clinical expertise.

Proactively assess and remind clients to complete surveys.

Provide quality patient care tailored to diagnosis, treatment, age, development, and identified needs; communicate findings to physicians and families; perform medical history, physical, and psychosocial assessment; communicate clearly.

Assist physicians and patients in understanding and navigating the MD Anderson referral process.

Use effective communication and interview skills to respond to information requests in line with policies.

Use standard medical and legal disclaimers to ensure appropriate interpretation of professional role.

Translate technical information to appropriate literacy level and verify understanding using motivational interviewing.

Participate in data collection and management processes using standard software.

Provide timely follow-up via call and e-mail for program-standard cases.

Handle difficult inquiries with communication skills to reduce anxiety and convey clinical expertise.

Conduct service recovery and utilize Patient Advocacy and HIS resources to assess satisfaction and needs.

Document appropriately in CONNECT program and electronic medical records.

Review team closure and response to alerts for timely support; provide care across disease processes and coordinate services.

Provide health education to patients and families (e.g., side-effect management, anticipatory guidance).

Knowledge Acquisition and Management

Acquire and demonstrate core knowledge of cancer and MD Anderson resources; utilize internal databases and approved resources.

Maintain knowledge of relevant outcomes, quality improvement initiatives, and institutional best practices.

Successfully pass ONCC certification within 1 year of employment.

Engage in continuing education and demonstrate mastery of content; advance computer and telephony skills.

Read new materials daily and integrate into operations; identify resource gaps and conduct research to address needs.

Mentor colleagues, participate in training, and contribute to team development.

Use call data to assess personal performance and align with departmental standards.

Promote certification requirements and conduct quality checks, reporting to leadership.

Customer Service and Quality Assurance

Assess client information needs using active listening and motivational interviewing; respond positively and timely.

Tailor responses and integrate solutions to meet client needs.

Confirm understanding and satisfaction before ending calls; problem-solve to meet needs.

Contribute to program quality assurance and apply new information as adopted.

Quality control review of call/e-mail documentation and materials.

Participate in periodic inquiries; use feedback to improve service.

Identify process improvements and educate team members in best practices.

Engage in ongoing learning and share innovations in clinical practice.

Team Involvement and Personal Leadership

Lead project teams and participate in Team Anderson activities.

Contribute to problem-solving, report obstacles, and lead departmental activities as assigned.

Adhere to MD Anderson and askMDAnderson policies; practice accountability.

Education Advanced Practice

RN Education Required: Master’s Degree in Nursing or Doctor of Nursing Practice. May be waived for applicants with BON recognition prior to 1/1/2003. Texas Board of Nursing requirements must align with clinical practice area and patient population. Preferred: ONCC or AANP and/or AOCNP certification.

Physician Assistant

Education Required: Graduate of a PA program approved by the AMA Council on Medical Education.

Experience Experience Required: Minimum 2 years oncology experience in hospital or clinic; experience speaking with patients and families by phone; proficient in multitasking and computer software (Microsoft Office, internet). Preferred: 5 years; telephone help line, counseling, teaching, or customer service in oncology or population health; bilingual Spanish/English.

Licenses/Certifications APN: BON authorization to function as APRN; Required: limited prescriptive authority by BON; compliance with QA initiatives and certification assessments. PA: NCCPA certification; Texas license; Required: limited prescriptive authority by the State; BLS mandatory; ACLS/PALS optional.

Education/Experience: See above; two years APRN experience in oncology recommended. Licenses: BON or TSBPAE as applicable; BLS/CPR required.

May be required: Eligibility for limited prescriptive authority in Texas.
