Shift Hours : 19 hours per week. Saturdays, 9:00am - 7:30pm. Sundays, 9:00am - 6:30pm. Weekend Rotation: 5 out of 6 weekends (Saturday - Sunday) in a 6-week schedule set. One weekend off in a 6-week scheduled block. Staff will be scheduled during the weekday during their off weekend. Holiday Commitment : 1 major bundled holiday. 3 minor holidays. Considered part of the ride out team and essential commitment to show up to 1MC. Mission

Position Summary
The primary purpose of the Clinical Programs of askMDAnderson is to provide transitions in care and after-hour triage via phone and online communication for patients, family, and caregivers after they have visited the MD Anderson Cancer Center. The programs support patients’ access to care in evenings and weekends and serve as the frontline healthcare resource for clinical support. This position requires strong knowledge of cancer care, excellent communication skills, motivational interviewing, and knowledge of MD Anderson services and resources. Shifts vary; Per Diem APP coverage primarily weekends (Saturday & Sunday) including evenings, nights, and holidays as needed. The APP must maintain clinical practice to sustain licensure, certification, and knowledge competency to provide safe, high-quality patient care. Key Functions
CONNECT discharge phone call program: Responds telephonically or via online communications to clinical alerts (medication questions, care instructions, follow-up appointments, post-discharge DME, home health, or other resources) to prevent readmissions or emergency visits. Close care gaps using approved resources and oncology expertise. Perform proactive assessments and remind clients to complete surveys. Provide quality patient care related to diagnosis, treatment, age group, development, and identified needs. Communicate findings to physician and family. Conduct medical history, physical examination, and psychosocial assessment. Communicate clearly and verbally. Practice autonomously within the scope of professional nursing practice; assist physicians and patients with understanding and navigating the MD Anderson referral process. Use comprehensive communication and interviewing skills to respond to information requests per askMDAnderson and MD Anderson policies. Utilize standard medical/legal disclaimers to ensure appropriate interpretation of professional role. Translate technical information to the client’s literacy level; use motivational interviewing and active listening to verify understanding. Participate in data collection and management processes using standard or customized software; ensure accuracy. Provide timely call and e-mail follow-up; manage inquiries from challenging customers with effective communication to reduce anxiety and convey clinical knowledge. Conduct service recovery and use Patient Advocacy and ask MD Anderson HIS resources as needed. Document in CONNECT program and electronic medical record. Review team closure and response to alerts for timely response and accuracy; provide health care throughout the disease process and coordinate services. Provide health education to patients and families (e.g., management of side effects, anticipatory teaching). Acquire and manage knowledge of cancer and MD Anderson resources; utilize internal databases. Maintain knowledge on quality improvement outcomes and institutional best practices. Pass ONCC certification within 1 year of employment; participate in continuing education and demonstrate mastery of content. Utilize computer and telephony equipment effectively; read and integrate new information into program operations. Identify program resource gaps and research solutions; support colleagues with difficult inquiries and participate in change initiatives. Assist in new employee training and continuing education programs as appropriate. Use performance data to assess personal work performance and align with departmental standards. Act as a resource for certification requirements and promote to team members. Perform quality checks with Project Manager; prepare quarterly reports for Director and Patient Experience Team. Contribute to quality assurance and service improvement initiatives; refine processes based on feedback and coaching. Seek opportunities to broaden knowledge and engage in professional organizations; demonstrate lifelong learning. Leads project teams and participates in Team Anderson activities; adheres to policy and just culture principles. Participates in annual evaluations and professional development; strengthens practice and partnerships to improve client outcomes. Other duties as assigned. Core Competencies

Required: Master’s Degree in Nursing or Doctor of Nursing Practice. May be waived for applicants with Texas BON recognition prior to 1/1/2003. Graduate education must align with clinical practice area and patient population. Preferred: ONCC or AANP certification and/or AOCNP. Physician Assistant Education
Required: Graduate of a Physician Assistant program approved by the AMA. Experience
Experience Required: Minimum 2 years oncology experience in hospital or clinic; experience speaking with patients and families via phone. Proficiency in multitasking with computer software (Microsoft Office, internet). Preferred: 5 years experience; telephone counseling or customer service in oncology or population health; bilingual Spanish/English. Licensure/Certification
APN: BON authorization to function as APRN; PA: NCCPA certification and TX licensure. Must have limited prescriptive authority as required by TX; BLS required; ACLS/PALS may be optional. Education/licensure requirements per Texas rules and MD Anderson policy. This position may involve security/infrastructure screening per state law.
