Responsibilities
Accurately assesses patient conditions and documents effectively to support charges and to communicate with other providers.
Provides comprehensive patient assessment, including functional, mental, social and physical conditions.
Establishes accurate diagnosis and seeks physician consultation as needed to confirm proper assessment.
Involves patients and caregivers in treatment planning and serves as an interface between the clinic and other providers to assure timely, comprehensive services.
Independently develops and implements treatment plans and evaluates patient outcomes.
Communicates patient needs, care plans and problem resolution clearly to physicians and health care teams.
Provides supervision to clinical staff as needed and helps develop/maintain policy and procedure for patient care activities.
Uses process improvement skills to contribute to team development and program growth.
Contributes to mission delivery and ensures patient care meets facility quality standards and patient satisfaction targets.
Educates patients on clinical and managed care aspects and participates in clinic and network task forces as appropriate.
Functions as an effective clinic team member and meets productivity standards while using clinic financial and performance information.
Qualifications
Active KY License
Cardiology/EP experience preferred or AcuteCare trained if APRN
