Description Outpatient Primary Care APP (PA/NP) – Western, NC UNC Caldwell is the choice for health care in Caldwell County. The Emergency Department sees approximately 30,000 visits a year. UNC Physicians Network provides the majority of primary care and select specialty care to our market from thirteen separate medical park locations throughout the county. Our 137 bed hospital is a member of the UNC Health System located in the Greater Hickory Area of Western, NC.

Position Features:

Location: Rivercrest Medical Park – 160 River Bend Drive, Granite Falls, NC 28630

Hours: Monday through Friday, 8 am to 5 pm

36 patient contact hours per week

Seeking an NP or PA who will assess and develop treatment plans for patients with acute and chronic illness. Prior Primary Care Clinic experience is required.

Summary

A Nurse Practitioner functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients within the APPs scope of practice in collaboration with a supervising physician. The APP reflects the mission, vision, and values of the organization, and complies with all relevant policies, procedures, guidelines and other regulatory and accreditation standards.

Responsibilities

Clinical

– Obtain health and medical history, perform comprehensive physical assessments, interpret diagnostic tests/results, develop differential diagnoses, and create treatment plans including prescribing/ordering appropriate evidence-based pharmacologic agents. Schedule follow-up, participate in hospital initiatives, conduct preventive health screening, maintain clear medical records, and ensure appropriate prioritization and patient flow. Maintain high productivity and lead or participate in quality improvements. Perform medical procedures per guidelines and privileges.

Education

– Educate patients and families about conditions, treatments, and follow-up; collaborate with teams to promote continuity of care; develop educational materials; provide education to physicians, staff, and students; pursue self-learning needs and attend relevant programs.

Leadership

– Provide instruction and mentorship, participate in community/outreach programs, and lead initiatives to improve healthcare delivery.

Professional

– Interact with patients and families respectfully; collaborate with coworkers; respond to needs promptly; share responsibility for workload.

Quality/Research

– Engage in daily quality improvement; participate in problem-solving and decision-making; lead interdisciplinary teams; develop and revise protocols; contribute to clinical program development.

Qualifications

Education Requirements:

Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program. Licensure/Certification Requirements:

Licensed as an Advanced Practice Provider (Nurse Practitioner, Physician Assistant, Clinical Pharmacist Practitioner, Certified Nurse Midwife, or Certified Registered Nurse Anesthetist) in North Carolina. Board certification if required by the Department. DEA license. Professional Experience Requirements:

No prior experience required. Knowledge/Skills/Abilities:

Demonstrated clinical competency; effective leadership and interpersonal skills; ability to work independently and with teams; strong communication and analytical abilities; ability to document for accurate billing.
