Hourly cases range anywhere from 4-12 hours at any time in the day, evening or night. Hartford HealthCare Independence at Home enables elderly and disabled individuals to achieve maximum independence and live with dignity while receiving quality care in their own homes. Our staff of highly trained professionals treat patients holistically, considering family and living situations for the most appropriate care plan. Responsibilities
Provide personal hygiene and grooming care to clients; assist with toileting, ambulation, and related activities. Provide meals to meet clients’ nutritional needs. Report changes in clients’ condition to nurse, supervisor, or client care coordinator. Perform routine housekeeping and home management duties. Record each client’s care on the care plan and submit care plans in a timely manner. Assist with emotional support for clients and their families. Assist with routine therapy exercises. Participate in case conferences as requested and attend required in-service education sessions. Use computer software and smartphone for required education. Represent the company professionally with a high level of customer service; think critically and escalate when appropriate. Qualifications
Two years of continuous work experience. One year of previous patient care experience; specific experience may be required by work area. New Grad CNAs are welcome. Current and valid Driver’s License in good standing with reliable transportation and proof of auto insurance. Ability to work independently and as part of a care team. Employment details
