Advanced Practice Provider (NP/PA) - Medicine Cardiology

UNC Cardiology has a great team of APPs, and we are looking to add to it. Both NPs and PAs are welcome to apply to any of our APP positions.
This particular position is referred to as our ACPW (Alternate Care PathWays) APP. ACPWs quickly facilitate and move patients to safe, less expensive sites of service, while still delivering high quality care and avoiding potential risks associated with hospitalization. In some cases, patients present at the ED (Emergency Department) who need to be seen quickly but do not necessarily require hospitalization. These are patients with specific, moderate acuity conditions who are medically stable. This APP role helps reduce unnecessary inpatient admissions and readmissions.
The ACPW will receive referred patients/appointments from the UNC EDs at Chapel Hill or Hillsborough hospitals, for timely evaluation (within 72 hours) in one of our Cardiology clinics (Eastowne in Chapel Hill or UNC Hillsborough Medical Office Building). The APP may also see other Cardiology patients such as Chest Pain, AF transition, hospital follow-ups, and potentially others as appropriate. A portion of time may be devoted to quality improvement related to access (e.g., meetings, coordination with colleagues). Our clinic APPs back each other up, so the successful candidate should be flexible in giving and receiving help as needed. Each APP has a supervising physician but also works independently with their own clinic template and schedule. The clinic schedule is weekday daytime hours (M–F 8–5).
Responsibilities
Clinical – Obtain relevant health and medical history, perform physical assessment, review and interpret diagnostic tests/results, develop differential diagnoses, and implement treatment plans including prescribing/ordering appropriate evidence-based agents consistent with guidelines and privileging; schedule follow-up management. Contribute to quality improvement and preventive health screening. Maintain comprehensive, clear records and up-to-date knowledge of disease processes and specialty literature. Develop and revise plans of care as patient needs change. Prioritize work and support staff to ensure efficient patient flow and productivity. Lead or participate in productivity-improvement projects. Perform medical procedures within guidelines and scope of responsibility.
Education – Educate patients and families about medical conditions, treatment, and follow-up. Collaborate with clinicians, staff, and students to promote continuous, patient-centered care. Develop educational materials and provide education to physicians, housestaff, nursing, ancillaries, and students. Pursue self-learning needs and participate in identified education programs.
Leadership – Provide instruction to physicians, housestaff, nursing, and students (precepting, shadowing, lectures, training, CE, rounds, morning report, meetings). Participate in community/network outreach programs and lead or develop these programs. Apply understanding of healthcare contexts to improve delivery and lead system improvements.
Professional – Interact with patients and families in a respectful, courteous, and timely manner. Collaborate with coworkers and staff to support patient care; respond promptly to needs and take responsibility for outcomes and workload sharing.
Quality/Research – Engage in daily quality improvement, identify and implement process improvements, and consult with management on clinical or system issues. Lead interdisciplinary teams to deliver integrated care, update protocols to reflect evidence-based care, influence policy, and develop new clinical programs to enhance patient care.
Other Information Education Requirements: Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program.
Licensure/Certification Requirements: Licensed as an Advanced Practice Provider (e.g., Nurse Practitioner, Physician Assistant, Clinical Pharmacist Practitioner, Certified Nurse Midwife, Certified Registered Nurse Anesthetist) in the state of North Carolina. Board certification if required by the Department. DEA.
Professional Experience Requirements: No prior experience required.
Knowledge/Skills/Abilities Requirements: Demonstrated clinical competency; effective leadership and strong interpersonal skills. Ability to work independently and collaboratively. Strong written and verbal communication and analytical skills. Effective interaction to ensure patient care; documentation to substantiate accurate billing.
