Social Services Supported Community Living Specialist role at Nishna Productions, Inc. This is a full-time position in Red Oak, IA. Hours are Monday–Friday, 8am–3pm. Responsibilities

The Supported Community Living Specialist (SCLS) reports to the Team Leader and the Director of Residential Services. Teach, train, and assist individuals in developing skills to perform activities of daily living as independently as possible. Coordinate health and medical needs and train individuals to manage their own health and medical needs. Develop or maintain skills for better participation in the community and assist individuals in accessing community services. Assist individuals in advocacy to ensure rights, needs, interests, and choices are met. Prepare individualized diet plans as needed and write Individualized Intervention Plans for each individual served. Provide emergency/safety skills training and training in budgeting and personal business management. Provide transportation to appointments and activities as needed and train in daily living skills. Orient new individuals, assess skills, and contribute to progress reports and the Person-Centered Plan (PCP). Supervise goals and collect data related to those goals; supervise funds for budgeting as necessary. Maintain a supportive and therapeutic atmosphere; supervise care and use of personal property; assist with purchases when needed. Train new staff on duties, schedules, policies, and regulatory procedures in coordination with leadership. Perform duties per the assigned work schedule and ensure individuals are never left unsupervised in twenty-four-hour care settings. Complete records and documentation using proper forms; attend staff meetings and in-service trainings. Ensure residences meet cleanliness, safety, and health standards and stay informed of programming and administrative changes. Maintain confidentiality of all information and document behaviors or contribute to intervention plans as needed. Handle other duties as assigned by Team Leader, Lead Staff, and Director of Residential Services. Monitor and document physician-ordered diet as needed and assist individuals with activities of daily living, including meal preparation, laundry, housekeeping, toileting, and bathing. Ensure clients are safe from environmental hazards. Essential Functions

Provide quality care and transportation; train in daily living skills (meal preparation, laundry, cleaning, personal hygiene). Maintain housekeeping standards; work with individuals who may have behavioral challenges or aggression; provide a supportive and therapeutic atmosphere. Administer medication and maintain documentation after training; exercise sound judgment for immediate decisions. Work according to the assigned schedule; duties may vary with overnight shifts and emergencies; complete documentation per agency requirements. Work independently to ensure welfare and safety of persons served; comply with state and federal HIPAA regulations. Understand PHI access levels and maintain confidentiality; safety and security clearances are part of role as defined by agency policy. Assist individuals with activities of daily living as listed above and ensure safety in all tasks. Qualifications/Requirements

18 years of age; high school diploma or GED; prefer previous related experience in teaching cooking, cleaning, and personal hygiene skills. Must obtain and maintain a valid Iowa Chauffeur’s license and have a driving record acceptable to the agency. Successful completion of pre-employment physical, TB test, and drug screenings; periodic physicals and TB testing thereafter; random drug screening. Complete a 10-hour medication-management class; willingness to work with individuals who may have behavioral disorders and aggressive outbursts. Ability to work with others as a team; good verbal and written communication skills; self-motivation. No legal history of assault, abuse, or bodily injury; willing to participate in agency investigations; confidentiality respected. Willingness to travel up to 70 miles as required. Physical Requirements

Ability to travel between locations; ability to enter/exist resident homes; capacity to walk stairs with materials; carry up to 50 pounds; provide physical care as needed. Ability to assist individuals in wheelchair transfers, assist with showers/baths, and perform routine household tasks (vacuuming, cleaning, meal prep, clothing care). Ability to perform snow removal as necessary for safety. Job Details
