ADVANCE PRACTICE PROVIDERS – SOUTHWEST GENERAL MEDICAL GROUP (SGMG) Southwest General Medical Group, Inc. – Primary Care
focuses on providing excellent quality medical care for family members of all ages, from newborns through seniors. Our board-certified physicians offer compassionate, comprehensive care, including preventive medical care, as well as acute and chronic care services. They diagnose and treat illnesses, provide routine checkups, health-risk assessments, immunizations, and screening tests. Our family practice physicians provide personal care and attention to each patient, working collaboratively to help maintain a healthy lifestyle and ensure optimal health and well-being.

HOURS:
Full-time, Monday–Friday, varied hours
POSITION SUMMARY The Advanced Practice Provider is a licensed professional who functions independently but provides care under the supervision of the Medical Director. They plan, implement, and evaluate treatment according to patient care standards and physician orders. They act as a liaison between the physician, patient, and family, and collaborate with other healthcare professionals as appropriate. They may assist in orienting new employees and participate in hospital or department committees.
MINIMUM QUALIFICATIONS Education:
Nurse Practitioner: Graduate of an accredited school of nursing in Ohio.
Physician Assistant: Graduate of an ARC-PA accredited physician assistant program. Master’s degree preferred or eligibility for prescriptive authority under Ohio law.
Experience:
Minimum of two (2) years of experience in a physician office or hospital setting; clinic experience is a plus.
Strong organizational and communication skills, both in person and on the telephone.
Experience in the indicated specialty area is preferred.
Licensure/Certification:
Nurse Practitioner: Current licensure by the Ohio State Board of Nursing.
Physician Assistant: Current PA licensure by the Ohio Medical Board.
