Your Role
As the
Outreach Worker
will work with data specialists to identify those patients lost to care. The goal is to re-engage patients who have not met with a provider after 6 months of their last visit.

Qualifications
Proficient in Electronic Health Record, Microsoft Word and Excel Exceptional organizational and interpersonal skills, with attention to detail required; strong oral/written communication skills are a must High school Diploma or GED preferred

Compensation
Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
