Summary Identify and provide for each resident’s social, emotional and psychological needs and facilitate the full development of the resident’s potential during their stay and assist in discharge planning.
Essential Duties & Responsibilities
Manage department staff including hiring, training, coaching, counseling, and termination.
Administer corrective action fairly and without regard to protected classes in accordance with federal, state, and local laws.
Complete Social Services portion of the MDS, Resident Assessment Protocols, Resident Care Plan, Progress Notes, and Social Services Assessment accurately and timely.
Assist residents and families with social, emotional, and family issues.
Participate in resident care planning by identifying the social and emotional needs of residents in accordance with the medical assessment.
Maintain progress notes for each resident as required by company policy and regulations.
Make referrals to support agencies as determined by the interdisciplinary team and keep records of such referrals.
Participate in the development of discharge plans and prepare residents for transition.
Serve as a general information source for families, residents, and the public.
Communicate with families or friends, utilize community resources, and inform residents and legal representatives of Residents’ Rights.
Coordinate logistics and content for monthly Family Council Meetings.
Coordinate resident room changes.
Invite residents or legal representatives to care plan meetings to evaluate the resident.
Maintain contact with residents, legal representatives, and other approved family members and provide information on the resident’s status.
Work with the Activity Coordinator to provide requested services for residents lacking family support.
Promote favorable working conditions and relationships with administration, medical staff, physicians, consultants, and other departments.
Work closely with the facility mental health provider to ensure all resident psychosocial needs are identified, referred, and served.
Assist in community education regarding aging, residents’ rights, and facility services.
Orient residents to the long‑term care environment and facilitate adjustment upon placement.
Perform other special projects and duties as assigned.
Job Requirements
Bachelor’s degree in social work, sociology, psychology, or a related human services field.
One (1) year of supervised social work experience in a health care setting working directly with individuals.
Effective verbal and written English communication skills.
Highest level of professionalism and confidentiality.
Ability to communicate at all levels of the organization and work well within a team environment.
Customer service orientation with the ability to work well under pressure.
Strong attention to detail and excellent organizational skills.
Strong analytical and problem‑solving skills.
Ability to work with minimal supervision, take initiative, and make independent decisions.
Approachable, flexible, and adaptable to change.
Function independently and maintain personal integrity with stakeholders and vendors.
