Position Title
Advanced Practice Provider - Family Medicine Bend East Clinic
Position Overview
Under the supervision of the Clinical Division Director and a designated physician, the APP provides medical and outpatient care for patients of all ages. Responsibilities include evaluation, diagnosis, treatment, documentation, and coordination of care across disciplines to achieve the best outcomes. APPs design treatment plans that may incorporate Behavioral Health, Care Management, therapy, pharmacy, nutrition, or surgery, and monitor patient progress, referring to specialists as appropriate. Work Schedule
Minimum 40 hours per week, with at least 36 hours of direct patient care. Scheduled days may vary with clinic hours. Call Expectation
Participation in the clinic call rotation, including evenings, weekends, and holidays, shared equitably with other providers. A Family Care physician will provide backup and support as needed. Key Responsibilities Provide comprehensive family medicine care across diagnosis, treatment, preventive care, and follow-up within the scope of licensure and credentialing.
Collaborate with Family Medicine, specialty providers, and referring clinicians to ensure excellent patient outcomes and a world-class experience.
Prescribe medications in accordance with SCHS policy, consulting pharmacists for polypharmacy patients and psychiatrists for psychotropic medications when appropriate.
Accurately document, code, and bill all patient encounters in the EHR in compliance with SCMG standards.
Interpret and report diagnostic tests in a timely manner (≤72 hours for urgent, ≤7 days for routine).
Provide patient and family education on health maintenance, prevention, and use of prescribed treatments.
Support the delivery and improvement of quality health measures.
Participate in after-hours call responsibilities.

Keep all licenses, certifications, and privileges current and unrestricted.
Education
Required:
Master of Science in Physician Assistant Studies or Master of Science in Nursing
Completion of annual SCHS-required education modules and CME to maintain licensure and board certification
Preferred:
Doctorate of Science in Physician Assistant Studies or Doctorate of Nursing Practice
Licensure/Certification/Registration
Required:
Unrestricted PA or NP license
NCCPA certification (PA) or AANC/ANNP certification with Family Medicine population focus (NP)
Ability to establish a Practice Agreement with a supervising physician (PA)
Current AHA BLS certification (ACLS required if providing sedation)
Unrestricted ability to participate in Medicare/Medicaid and other government programs
Active hospital privileges at SCHS
Preferred:
Oregon PA or NP license
Experience
Required:
Outpatient Family Medicine practice experience
Proficiency with EHR systems
Strong communication skills and ability to work effectively with a diverse population
Experience collaborating in team-based care models
Preferred:
Two or more years in active Family Medicine practice
Familiarity with the Medical Home model and quality measures
Experience with Medicare Annual Wellness Visits and outcome-based data review
Epic EHR experience
