Advanced Practice Provider (NP/PA) - SOM Amb Care-APCF-Neurology
Coloradoacc, Chapel Hill, North Carolina, United States, 27517
Overview
Become part of an inclusive organization with over 40,000 teammates, whose mission is to improve the health and well-being of the unique communities we serve. The UNC Neurology Neurogenetics program is looking for an Advanced Practice Provider (APP) to join our growing interdisciplinary group. Our team includes neurologists, genetic counselors, geneticists, nurses, and clinic staff. We collaborate with multiple clinical Centers of Excellence and interdisciplinary clinics, including the ALS Clinic, Hereditary Neuropathy Foundation CoE, Muscular Dystrophy Association interdisciplinary clinics, National Ataxia Foundation CoE, and Neurocutaneous disorders clinics. The Epilepsy Neurogenetics Initiative (ENGI) and UNC Huntington Disease Program clinics are co-homed in Neurogenetics and pediatric neurology/epilepsy and movement disorders, respectively. We also collaborate across departments to support care for neurogenetic disorders that can impact other systems such as cardiac and endocrine. We share long-term care of mitochondrial and metabolic disorders patients of all ages with clinics in pediatric genetics, and receive direct referrals from newborn screening programs. The APP will be involved in all aspects of patient care and education, including developing and enacting personalized symptom management care plans that leverage medical, surgical and rehab options; facilitating collaborative care across neurology divisions and other departments; discussions around advanced directives, hospice/palliative care, and other end-of-life issues. Neurogenetic disorders seen in our clinics range from common to nanorare, and new genetic diagnoses are constantly discovered. The APP will be closely involved in finding and sharing evidence-based clinical practices across our group and with collaborating providers, especially for very rare or newly described disorders. Ability and willingness to engage with lifespan care are key attributes for this position. UNC Neurogenetics clinics see patients of all ages. This APP position will mainly support adult patient populations, but clinic visits with teens are not uncommon and visits with older children are possible. This is a "mirror image" of the ENGI clinic, and the clinics homed in pediatric genetics (mitochondrial, metabolic disorders) mainly support pediatric patients but will see older patients when clinically applicable. In addition, a neurogenetic diagnosis in one family member may immediately impact multiple other people in both older and younger generations. Comfort with addressing whole family needs and coordinating care across traditionally pediatric, adult, and geriatric clinic settings is important. UNC Neurogenetics is a growing program within the larger context of genetics and genomics care at UNC Health. The APP will contribute to program development, such as division and department efforts to expand subspecialty neurology care access across NC, new neurogenetics clinic registries, outreach to primary care providers and engagement with UNC Health system new initiatives, for example new genetics care referrals triage pathways, access to new genetic therapeutics.
