Job Description
Nurse will: Provide direct patient care in home setting taking role as case manager Work in multi-disciplinary team to provide full range of home health services including assessing, planning, coordinating and evaluating plan of care under the orders of a physician Participate in team-based quality improvement, supervises C.N.A. Access to protected health information is limited to the minimum necessary to perform the duties of the job. Qualifications
Current Nursing license from the State of IL Current BLS Valid Illinois driver’s license with dependable transportation Proof of auto insurance Ability to work independently, make sound decisions and proficient in computer skills. Proficient in IV’s insertion, lab draws and works effectively in a team atmosphere Excellent communication skills and portray a positive attitude. Able to lift up to 70 pounds occasionally and have the ability for frequent stooping, kneeling, bending, twisting and turning Position requires continuous walking and standing Minimum of 2 years’ experience in an acute care setting with home health experience preferred
