Position Overview We are seeking a full-time Nurse Practitioner or Physician Assistant to join our community-focused outpatient family medicine clinic in Dewey-Humboldt, AZ. This is an excellent opportunity for providers who value long-term patient relationships, a supportive team culture, and a predictable weekday schedule. With no nights, weekends, or on-call duties, this role allows you to focus on high-quality, patient-centered care.

Monday–Friday schedule with no evenings, weekends, or on-call
Relationship-centered practice with a manageable patient panel
Dedicated medical scribe support to reduce administrative burden

Compensation & Benefits
Salary:
$120,000 – $150,000 annually (based on experience)
Incentives:
Performance-based bonus plan

Key Responsibilities
Perform annual wellness exams, preventive screenings, and Medicare visits
Diagnose and manage acute conditions and minor injuries
Provide chronic disease management (diabetes, hypertension, COPD, etc.)
Order, interpret, and follow up on diagnostic tests and imaging
Educate patients on medications, treatment plans, and healthy lifestyle practices
Maintain accurate EHR documentation with synchronous scribe support
Coordinate referrals and manage transitions of care
Participate in clinical quality initiatives and practice improvement efforts
Qualifications
Active and unrestricted Arizona NP or PA license
Current DEA registration or ability to obtain
Credentialing eligibility with Medicare, Medicaid, and commercial insurers
No history of malpractice or disciplinary action
Graduate of an accredited U.S.-based NP or PA program
Note: Puerto Rico-based programs are not accepted
