We are seeking a Lead Physician located within surrounding NJ counties: Mercer, Middlesex or Monmouth to support our community-focused healthcare services. As a Lead Physician (MD/DO), you will: Oversee care and outcomes for a patient cohort, managing directly and in collaboration with nurse practitioners. Coach and support nurse practitioners in your area, ensuring clinical excellence and professional development. Meet key metrics, such as enabling patients to stay at home whenever possible. Conduct home and telehealth visits as appropriate. Actively participate in Interdisciplinary Team (IDT) Meetings, contributing medical expertise to shape care plans based on:

Patients' prognoses, disease trajectory, and goals of care. Primary drivers of hospitalization/ED risk and interventions to mitigate them.

Drive high performance on CMS STARS and NCQA HEDIS quality metrics. Deliver evidence-based care, adhering to clinical protocols. Coordinate with PCPs, specialists, home health, hospice, and other providers as needed. Determine the need for specialist consultations, deprescribing, or care in emergency/hospital/skilled nursing facilities. Provide medical administrative support, including signing required forms. Complete timely, accurate clinical documentation in accordance with practice standards. Collaborate with the Medical Director and Practice Manager to drive quality improvement, operational enhancements, and care delivery evolution. Participate in the on-call rotation as determined by the Medical Director. Represent the practice positively in the medical community and uphold Clover’s values. You have an MD/DO degree and are Board Certified/Board Eligible in Family Medicine, Internal Medicine, Geriatric Medicine, or Hospice & Palliative Medicine (required). You have an active New Jersey medical license, CDS, and DEA (required). You have prior experience in house calls, fee-for-service, and/or value-based care (preferred).
