Harris Health is designated for nursing excellence and collaborates with Baylor College of Medicine, UTHealth, MD Anderson and other institutions. Responsibilities
The Nurse Clinician, Hospital at Home (NC- HaH) provides high-quality, at-home acute care and independently manages patients with problems common to their assigned clinical area, seeking assistance when needed. Coordinate, facilitate, and participate in the development and implementation of facility enrollment into the HaH patient system-wide using the appropriate eligibility criteria. Develop, organize, and facilitate patient-centered in-home care delivery by prioritizing patient needs and using a scheduling system for efficient, geographically organized home visits and point-of-care delivery. Access and apply evidence-based resources and professional standards related to delivery of HaH responsibilities. Participate in quality improvement projects, collect data for trends, identify problems, and apply findings to improve clinical outcomes. Model professionalism and promote a customer-friendly environment using Service First behaviors when interacting with patients, families, and staff. Minimum Qualifications
Degrees : Bachelor’s degree in Nursing. Licenses & Certification :
Registered Nurse in the State of Texas Related Specialty Certification as required by unit/service Basic Life Support (BLS) from AHA or Red Cross Current and unencumbered Texas Driver's License
Work Experience :
2 years of clinical inpatient experience or home health/community visits 1 year of home health/hospice and/or med-surge experience
Language : Bilingual skills (preferred) Communication Skills : Above-average verbal communication, exceptional public speaking, writing/correspondence, and writing/reports Proficiencies : MS Word, PC Other Special Requirements : Equipment operated includes computer and vehicle; HaH-specific requirements Work Attributes
