Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

Who You Are
You are a motivated Physical Therapist who shares our vision of delivering in-home care. You treat patients in a caring and compassionate manner and help them achieve their goals of staying in their own home whilst receiving treatment. You may be a recent graduate who wants experience in home health.
Schedule
Your schedule depends on your situation. We are open to part-time, full-time and per-demployment, and willing to adapt hours in the future. This allows you to build your own schedule, allowing you to create a comfortable caseload. You might want to supplement your current caseload or move into home health from your Outpatient, SNF, or hospice experience.
Travel
This role comprises 100% travel within our service region. You will be traveling to patients’ homes.
Service Region
We have several counties that we service across Northern California specifically Butte County, Shasta County and Yuba Sutter counties and surrounding communities.
Duties and Responsibilities
To perform this job successfully, you must be able to perform the following duties successfully: Evaluate patients upon physician diagnosis Develop appropriate treatment goals for each patient by assessing and identifying areas of deficits impacting posture, locomotion, strength, endurance, balance, coordination, joint mobility, flexibility, pain and functional abilities in daily living. Deliver physical therapy to patients in their own homes Instruct and education the patient/caregiver to promote the safe execution of the facilitatory techniques taught Ability to supervise Physical Therapist Assistants, as required
Essential Requirements
One year of experience as a home health care provider Current California State license as Physical Therapist Current CPR certification Valid California Driver’s License Eligible to be insured under our liability policy with a clean DMV report Your own car, and proof of current auto insurance
Required Skills and Abilities
Demonstrated technical competence and accountable for work output. Demonstrates proficiency in performing physical therapy procedures and modalities and in assessing the effects of these Demonstrates sensitivity with patients, family, and their medical team Ability to work with others, at all levels within the organization, and collaborate effectively. Above-average interpersonal, problem-solving, and written and oral communication skills. Professional behavior when on duty Ability to maintain patient records and able to organize daily schedule
Physical Requirements
Sits, stands, bends, lifts, walks, and moves intermittently during working hours. Able to lift 50 pounds without assistance. Able to drive intermittently daily
Compensation and Benefits
Pay range is between $55.00 - $70.00 per hour. Exact compensation may vary based on skills, experience, and location. You will be reimbursed for the mileage you travel from the office to participant’s homes. You will receive a monthly phone stipend. On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, long term disability insurance as well as Flexible Spending Account (FSA). We offer 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.
