We are hiring for an RN Case Manager in Hospice.
This is a Full-Time position involving Home Visits for Hospice.

Responsibilities As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, continuously evaluating your performance and making necessary adjustments to improve productivity and quality of care. Conduct the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral. Identify the patient's/family's physical, psychosocial, and environmental needs and reassess as needed, at least every 15 days. Initiate and coordinate the plan of care, maintaining accurate clinical notes regarding the patient's condition. Document problems, goals, interventions, and patient/family responses to hospice care. Collaborate with the patient/family, attending physician, and other IDG members to provide daily patient and family care. Education and Experience Experience Requirements One year of clinical experience. License Requirements Current RN licensure in the state of practice and one year of clinical experience.
