Overview
The Clinical Pharmacist – Pharmacy Quality will help support the Monogram Health Pharmacy Department’s Quality program through completion of standardized audits of completed clinical pharmacist work (CMRs, etc.). He/she will also help create and deploy interventions to positively impact Medicare STARs / HEDIS Quality Metrics. This pharmacist will also contribute directly to Monogram’s medication therapy management (MTM) program through completion of comprehensive medication reviews (CMRs) and associated patient and provider outreach to implement recommendations that optimize patient care. Finally, he/she will assist with maintenance and reporting of clinical and operational quality metrics and Medicare STARs / HEDIS Quality Metrics, and support follow up actions including individual and/or group education and performance / process improvement.
Roles and Responsibilities
Pharmacy Quality Program - Time Allocation: 40%
Support the development and maintenance of departmental policies and standard operating procedures (SOPs)
Support the development and maintenance of clinical pharmacy practice priorities, standards, and resources that support pharmacist in daily clinical practice and are consistent with Medicare STAR s/ HEDIS Quality metrics and regulatory requirements as appropriate.
Perform audits of completed clinical pharmacist work (CMRs, etc.), evaluating for adherence to departmental policies, SOPs, and clinical quality standards
Conduct maintenance and reporting of individual and departmental quality metrics
Support during internal and external audits of Monograms Pharmacy Department processes.
Work collaboratively with Monogram Quality and Compliance team to ensure pharmacy program meets client deliverables and regulatory requirements as appropriate.
Direct Patient Care - Time Allocation: 40%
Perform and write concise patient-specific comprehensive medication reviews (CMRs) consistent with Monogram productivity and quality standards.
Perform targeted medication reviews (TMRs) focusing on patient disease processes and medication use patterns that contribute to CMS STARS and HEDIS Quality Measures
Drive meaningful medication interventions that result in medical providers’ and patients’ adherence to evidence-based medication guidelines
Review patient care reports, clinical notes and other medical records as needed for drug therapy action plan development, targeted clinical outreach programs and quality
Monitor important medication use trends for patient populations under Monogram’s care. Stay current with evidence-based prescribing for individuals with kidney disease and related polychronic conditions.
Miscellaneous - Time Allocation: 20%
Assist with the development and delivery of clinical and operational training materials and sessions for Monogram staff both within and external to the Pharmacy Department, aimed at building a standardized, evidence-based, and effective MTM program for Monogram clinical staff.
Support Monogram and pharmacy leaders as necessary with any tasks required to deliver excellent personalized medication management services to our patients and perform all other duties as assigned. This may include timely responses to urgent needs. Therefore, a sense of flexibility and team-first approach are essential.

Qualifications
Bachelor’s degree in pharmacy or PharmD
Current pharmacist license
3+ years previous experience (or equivalent) actively practicing as a pharmacist, preferably within a managed care, ambulatory care, or clinical practice environment
Desired qualifications include:
PGY1 residency in ambulatory care, managed care, or related clinical field
BPS board certification in pharmacotherapy, geriatrics, ambulatory care, or related clinical field
Board certification in MTM
MTM certificate training from APhA, Powerpak, or similar program
Outpatient Medicare & Medicaid health plan experience through work at a health plan or a pharmacy benefit manager (PBM) supporting a health plan. This includes:
Internal and external audit experience
Experience with Performance Improvement Plans (PIPs) and/or Corrective Action Plans (CAPs) for people and processes
Working knowledge of Medicare Part D Pharmacy Program (benefits, formulary, MTM)
