Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist at Enhance Rehabilitation, you will be part of a growing team providing high-quality care for patients in SNF settings. In this role, you will evaluate and treat patients following individualized care plans.
Responsibilities include:
Providing excellent care for our patients Offering exceptional customer service to patients and their families Collaborating with facility staff to ensure optimal patient care Evaluating new patients, developing care plans, and setting measurable, functional goals Treating patients, addressing concerns, and educating them on their injuries, progress, and expected outcomes Supervising Physical Therapist Assistants Re-evaluating patient progress, revising care plans, and updating goals Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes
