Purpose: To develop, coordinate and implement programs and services for residents with cognitive impairment; to provide a therapeutic environment that maintains each resident's highest level of well being of mind, body and spirit.
Responsibilities
Direct and supervise Memory care staff. Coordinate the implementation of the Memory Care Program mission, goals and philosophy. Provide or secure training to associates in the Community including training caring for residents with memory impairment and other advanced training. Provide or secure the additional training/practicum experience to Resident Caregiver associates of Memory Care Program. Coordinate with Director of Nursing in establishing and maintaining a staffing pattern to meet our customized care commitment. Maintain a mentally, physically and spiritually stimulating activity program that is geared to the individuals preferences and capabilities. Participate with the Director of Nursing in the hiring, scheduling and supervising of Resident Associates assigned to the Memory Care program. Act as liaison to facilitate communications among Memory Care, the community, family members and the local community. Maintain an environment in the residents' personal and congregate areas, which will help to compensate for cognitive, sensory and physical impairments. Interview residents and families prior to admission. Coordinate the pre-admission screening and assessment process with Directors of Marketing and Resident Care. Coordinate with the Director of Nursing, Memory Care Program Caregivers and families in the development of the 24-hour plan for the residents and updates the plan in a timely manner. Coordinate delivery of services for each resident according to the 24-hour plan. Attend Department head meetings and lead regularly scheduled Memory Care Program meetings. Provide Caregivers with updates and in-service training programs and Memory Care programs. Pursue educational development in Alzheimer’s disease and related disorders. Assist the Marketing Director in promoting the Memory Care Program to doctors, hospital discharge planners and local community agencies. Participate as instructed in the sales process of potential residents. Coordinate the Memory Care Program volunteer/mentor program. Develop and maintain the community's resource library. Model our company philosophy, helping out whenever needed to meet residents' needs and working as a member of the overall team. Any other duties as assigned by the Executive Director and/or Director of Nursing.
