Charlotte Bay Rehab and Care Center is seeking an experienced and motivated

Lead MDS Coordinator (RN)

to join our leadership team. This role is ideal for a detail-oriented nurse with strong knowledge of the MDS and care-planning process who is passionate about ensuring the highest quality of care for our residents. What You’ll Do

Drive Quality Care:

Oversee and support the delivery of exceptional long-term care services.

Conduct Assessments:

Perform comprehensive initial interviews and resident evaluations.

Develop Care Plans:

Create individualized care strategies to support short- and long-term goals.

Lead Collaboration:

Facilitate interdisciplinary team meetings with staff, families, and caregivers.

Engage with Specialists:

Partner with providers to enhance resident outcomes.

Mentor & Educate:

Provide guidance to nursing staff and lead in-service training on MDS, RAPS, and Triggers.

Ensure Compliance:

Manage MDS and care-planning schedules to maintain regulatory standards.

What We’re Looking For

Active

Florida RN license

in good standing.

At least

3-5

years of experience

in a Skilled Nursing Facility (SNF).

Strong expertise in

MDS, Medicare regulations, and care planning .

Excellent leadership, communication, and organizational skills.
