Overview
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) for a Locums assignment with potential to convert to permanent in a primary care practice serving geriatric patients in a value-based care model. This is a hybrid role with approximately 40% coverage in our outpatient clinic and 60% home-based patient care.
Compensation Compensation:
$75 – $85 per hour
Position Details
Schedule:
Monday – Friday (2–5 days per week; flexible days that suit your availability)
Patient Load:
Home Care: 3–5 patients per day
Clinic: Small census of 7–10 patients per day
Setting:
Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patients
Responsibilities
Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care
Diagnose, treat, prescribe medications, and develop individualized treatment plans
Order and interpret diagnostic tests (labs, imaging, etc.)
Document accurately in the EHR system
Provide patient education and promote wellness initiatives
Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care
Refer patients to specialists and manage follow-ups
Ensure compliance with state, federal, and professional regulations
Requirements
Active NP or PA license in NY
Minimum 2 years of primary care experience
National Board Certification
Bilingual a plus
Medicaid and Medicare Provider Id required
