Summary/Objective:
The Case Manager is part of the clinical team providing ongoing care coordination and aftercare planning to individuals who are receiving residential services. The Case Manager provides comprehensive coordination of care to address clients’ service and support needs, including: advocacy; assessment; referrals and linkages to services and resources; goal setting; transporting clients to court and other appointments; coordination of care with outside programs, primary care providers, medical and behavioral health specialists; employment, housing and financial applications; aftercare and discharge planning; assistance with intake coordination; working closely with counselors/clinicians and recovery specialists to promote attainment of treatment plan goals; attending community based meetings
Essential Functions
Demonstrate ability to establish and maintain engagement of client toward meeting treatment plan goals.
Meets with clients weekly to provide care coordination and aftercare planning
Conduct assessments to identify clients’ needs, strengths and barriers to attend treatment plan goals
Conduct care planning in collaboration with clients and treatment team
Facilitates psycho-educational groups
Ensures that clients’ whole-health and social determinants of health needs are identified and addressed
Conducts post-discharge follow-up for clients
Facilitates and carries out referrals and linkages to all service needs that clients have
Demonstrates professional and respectful boundaries with clients
Collaborates with all service providers to fully address the needs of clients
Provides transportation as needed to any appointments/meetings in the community
Completes all required documentation in a timely manner and in accordance to Joint Commission, Dimock and regulatory standards
Coordinates discharge and aftercare planning well in advance of discharge to ensure that clients receive a fully continuum of care
Attend all staff meetings and trainings as assigned
Assist clinical team in crisis management
Returns all phone calls to providers calling within 24 hours of receiving phone
Adhere to all organization, federal and state regulations, guidelines and/or contractual obligations.
Maintains respectful, effective and professional working relationships with other Dimock staff.
Conducts post-discharge follow-up for clients.
Demonstrates working knowledge of Dimock’s continuum of care and facilitates appropriate referrals/contacts within the larger system.
Demonstrates knowledge of and adherence to Dimock’s Policies and Procedures.
Other duties as assigned.

Physical Demands
Light (requires lifting 20 lbs., with frequent carrying of up to 20 lbs.)
Position Type/Expected Hours of Work
This position will work part time on Saturdays and Sundays. More hours during the week can be considered. Shifts typically start between 7am and 11am.
Required Education
Bachelor’s Degree in human services, psychology, sociology, health services, or related field
At least one year working in a substance use progra
Preferred Education / Experience
At least two year working with individuals with substance use and mental illness; homelessness
Current enrollment in a human services related field for a Masters Program or a completed Masters Degree
