Job Description Summary Performs cardiac rehabilitation functions to facilitate patient cardiac fitness. Participates in process of assisting the patient with cardiac risk factor reduction. Essential Functions

Admission and Evaluation

Conducts intake evaluations as assigned to meet patient needs in accordance with established standards of care and practice. Evaluate patients to identify orthopedic, neurological or medical problems that may interfere with program completion and document findings. Develop a safe and effective exercise prescription based on GXT data and medical information. Complete admission data input into Qtel Monitoring System as needed and monitor patient compliance with the risk reduction plan weekly.

Treatment

Conducts exercise program with assigned patients according to established treatment plans and standards of care. Collaborates with team members to optimize treatment, using education materials and creative techniques. Lead sessions including warm-up, aerobic activity, resistance training as prescribed, and cooldown; monitor heart rate and RPE. Report patient progress, coordinate with Medical Director as needed, and participate in team meetings. Manage Community Maintenance exercise groups, including enrollment, supervision, and safety checks.

Documentation

Completes all documentation per department standards, including measurable goals and discharge recommendations. Document all aspects of team communication, assessments, patient responses, and teaching in appropriate records. Ensure discharge and post-program plans are documented and communicated to the Medical Director and RN as needed.

Discharge

Demonstrates timely discharge with appropriate documentation. Collaborates with patient and nursing staff to prepare for discharge and finalize discharge plans. Complete discharge charts within policy timelines and provide information for post-discharge exercise maintenance and risk reduction.

Billing

Enter daily charges accurately and timely; maintain billing accuracy with minimal errors. Assist in training others on billing processes and ensure compliance with modifiers and account selection.

Productivity

Demonstrate behaviors that support department productivity standards; understand the relationship between treatments and reimbursement.

Technology and Learning

Embrace technology to improve department processes and patient care; utilize Meditech/PCS and Qtel Monitoring System effectively. Engage in ongoing learning and be willing to adopt new applications and techniques.

Qualifications Note

BLS - Basic Life Support; knowledge of cardiac rehabilitation, exercise physiology, testing, prescription, group leadership, and monitoring techniques.
